Halcyon Care North West is partnering with a leading care provider to deliver high-quality support services to individuals with physical disabilities and complex health needs. We are seeking a dedicated and experienced Support Coordinator to join our team in Salford.

About the Role:

As a Support Coordinator, you will play a key role in providing respectful personal care and practical support to 10 service users, building their independence and enabling them to live meaningful and valued lifestyles. You will work across two services, supporting and supervising staff, and liaising effectively with service users, families, and multidisciplinary teams to ensure a consistent and coordinated service.

Key Responsibilities:

  • Provide personal care and support to service users, promoting their self-esteem, happiness, and emotional health
  • Assist the Registered Manager in the coordination and operational management of the service
  • Support and supervise staff, deploying the team according to service user needs and preferences
  • Deputise for the Registered Manager in all aspects of service delivery
  • Develop and maintain Individual Lifestyle Support Plans and Person-Centred Plans for service users
  • Ensure that service users have access to independent advocacy and advice
  • Promote a warm and positive approach to families and significant others
  • Ensure that service users receive all necessary advice, care, and regular health checks to maintain their physical wellbeing

Requirements:

  • Experience of working with individuals with physical disabilities and complex health needs
  • Strong leadership and management skills, with the ability to support and supervise staff
  • Excellent communication and interpersonal skills, with the ability to work effectively with service users, families, and multidisciplinary teams
  • Ability to work flexibly, including evenings, weekends, public holidays, and sleep-in shifts
  • Strong organisational and administrative skills, with the ability to prioritize tasks and manage multiple responsibilities
