Our client, a dynamic and forward-thinking organisation, is seeking a dedicated On Street Support- Housing Solutions to join their team. In this role, you will play a vital part in helping individuals with multiple and complex needs prevent a return to rough sleeping. You will provide early intervention, ongoing support, and guidance following discharge from the hospital.

What you'll be doing:

Offering guidance and developing support plans to ensure clients sustain their quality of life
Engaging with various support agencies to promote personal independence and well-being
Acting as the main point of contact for casework within the accommodation, coordinating solutions for drug and alcohol, domestic violence, and mental health issuesWhat we're looking for:

Demonstrable experience in case planning and complex case management
Sensitivity to the diverse needs of individuals from a range of backgrounds
Strong communication skills, both written and verbal
Knowledge of local social and health care services and the ability to navigate systems and pathwaysQualifications and skills:

Experience working independently and providing high-quality care
Proficiency in Microsoft Word, Excel, and Outlook
