A qualified Social Worker Team Leader is needed to cover while recruiting for a permanent role.

The role is to support the Hospital Discharge Team and Intermediate Care Team, with safe and timely discharges, covering Calderdale Royal Hospital and Huddersfield Royal Infirmary.

The role of Team Leader is to deputise for the Team Manager and assist in managing a Community team. The role is to provide professional support to the team and to support the Manager to ensure that resources, including staffing resources are effectively co-ordinated, deployed and managed. Whilst ensuring that the provision of care to meet assessed needs and outcomes is in accordance with statutory responsibility, policy and team objectives and service priorities and to work to safeguard vulnerable adults. The post holder will manage a caseload of complex cases

You must be able to attend the office at least 2-3 days per week.

30 hours a week

• DIPSW or equivalent qualification in Social Work
• Social Work England registration
• Enhanced and in date DBS certificate.
• Excellent communication skills and be able to make good decisions based on information available.
• Can work alone and within/contribute to a team/multi-agency set ups
• Possess knowledge of service needs.
• Competent IT skills and be able to navigate electronic file/recording systems.
• Can write clear, concise, and robust assessment/reports/files.
• Have a flexible approach to work and working hours when the need arises
